Public forms: website request form, review page and sign-in code
Every box on the visitor request form, the star-rating and private feedback page, and the two-step code box on the WordPress sign-in page.
These are the boxes your visitors and customers see. You do not fill them in, but you should know what each one does. Their settings live elsewhere (see the related articles).
1. Website request form (visitor side)
Where it appears: on the page where you put the shortcode [dreamscaper_request], the one-line embed, or the form page link. You turn it on under Contractor Hub → Settings & Integrations → Connected apps (see settings-connected-apps-request-form). It is off until you do.
Each request becomes a lead. It never merges with an existing customer by itself, the customer gets an instant reply, and your office is alerted.
| Field (exact label) | What it does for VisionScaper | What the visitor enters | Default / notes |
|---|---|---|---|
| Your name | The lead’s name. Required. | Full name. | Empty. |
| Where replies go. Needed for the email consent to count. | A valid email. | Empty. | |
| Phone | Where texts go. Needed (10 digits or more, valid number) for the text consent to count. | Phone number. | Empty. |
| Property address | The job address. Required. Suggestions appear after 5 characters. | Street address. | Empty. |
| Town | The town. Filled from a chosen suggestion if empty. | Town name. | Empty. |
| What do you need? (pick any) | The services requested. Shown only if you have services enabled for the form. | Tick any. | None ticked. |
| Anything we should know? | Notes saved on the request. | Up to 2,000 characters. | Empty. |
| Photos of the area | Pictures saved with the request. Shown only if your “Photos allowed” setting is above 0, with that number shown. | Image files. | None. |
| Day and time buttons | Self-booking. Shown only when self-booking is on and there are open days. The visit stays a request until confirmed (“we will confirm it”). | Pick a day, then a time. | None. |
| Best times to reach you (optional) | A note on when to contact them. | “Weekday mornings”. | Empty. |
Email me about this request (consent_email) |
Records consent to email about this request. Without it, no email is sent to the visitor. | Tick. | Unticked. |
Text me about this request (consent_sms) |
Records consent to text about this request. Without it, no text is sent. Text also needs your text messaging to be ready. | Tick. | Unticked. |
Hidden trap field (hp) |
A box people cannot see. If a program fills it, the request is treated as spam. | Nothing. | Empty. |
| Cloudflare Turnstile | A spam check, shown only if you switched it on. | Passes by itself in most cases. | Off. |
| Send my request (button) | Sends the form. A status line appears below. | Click once. |
The consent wording shown under the boxes is saved with the request, with the time, so you can show what the visitor agreed to. It is not marketing consent. The boxes are not ticked for the visitor.
2. Review request page (customer side)
Customers reach this page from the link in a review request message. It starts with “How did we do?”.
| Field (exact label) | What it does for VisionScaper | What the customer enters | Default / notes |
|---|---|---|---|
Star buttons 1 to 5 (r) |
Records the rating. 4 or 5 stars lead to “Thank you” and links to your public review sites. Under 4 stars lead to the private form. | Tap a star. | None. |
Anything we could do better? Tell us privately (textarea fb) |
Shown to 4 and 5 star raters, inside a closed section. Sends a private note to the owner. | Up to 3,000 characters; placeholder “Your note goes only to the owner”. | Empty. |
Your name (optional) (nm) |
Shown under 4 stars when the customer is not already known. | Name. | Empty. |
What could we have done better? (textarea fb, required) |
Shown under 4 stars. Sends a private note to the owner. | Up to 3,000 characters. | Empty. |
| Send privately / Send to |
Saves the note and shows “Your message went straight to …”. | Click. |
The page always keeps the public review links visible, even after a low rating. Private notes can be brought into Review replies (see article card-reader-and-review-replies-fields) with Bring them in.
3. WordPress sign-in page: Two-step code
| Field (exact label) | What it does for VisionScaper | What to enter | Default / notes |
|---|---|---|---|
Two-step code (only if you turned it on) (dscp_otp) |
Added to the normal WordPress sign-in form. After the password is checked, VisionScaper asks for your authenticator code (or a recovery code) if two-step is on. | The 6-digit code, or leave blank if two-step is off for you. | Empty. Set it up in security-screen-fields. |
4. A technical field: `organs`
organs is not a box on any screen. It is a field VisionScaper sends to Pl@ntNet when it identifies a plant from a photo. The value comes from a request setting named organ that accepts leaf, flower, fruit, bark or auto, and uses auto for anything else. No screen label for choosing it was found in code, so how it is chosen in the designer is (not verified in code).
Steps
- To preview the request form, open the page that holds the shortcode in a private browser window.
- Fill it in with a test lead, leave both consent boxes unticked, and send.
- Check Customers and Inbox for the new lead.
- To preview the review page, open the link from a review request you sent yourself.
Tips
- Consent boxes are never pre-ticked, on purpose.
- Keep the Turnstile off unless you receive spam.
If something goes wrong
- No form shows: it is switched off in Connected apps.
- No text arrived: the visitor must have ticked the text box and given a valid phone number.
- Sign-in rejects the code: use a recovery code or ask the owner to reset your two-step.
